This guide separates the reusable questionnaire contract from concrete questions and recorded answers.
| Layer | Published contract |
|---|---|
| Profile | Defines reusable structure, invariants, and mapping conventions for a class of questionnaires or responses. |
| Questionnaire instance | Gives one concrete set of questions a canonical URL and version. Each published instance is classified below as an anchor identity, example, or maintained definition. |
| QuestionnaireResponse | Records answers and identifies the exact source questionnaire as url\|version. It does not redefine the questionnaire. |
HL7 SDC supplies the targetStructureMap mechanism used by the typed Job
profile. The mapping contract remains separate from questionnaire wording.
| Questionnaire | Classification | Publication meaning |
|---|---|---|
e61-rehabilitation-assessment |
Anchor identity | Minimal versioned identity for e61 with self-authored items. It is not a form reproduction. |
e62a-potential-assessment |
Anchor identity | Minimal versioned identity for e62A with self-authored items. It is not a form reproduction. |
example-anamnese-erstanamnese |
Example | Worked Anamnese example that demonstrates the profile. Its wording is not mandatory practice content. |
q-upper-arm-injection |
Example | Worked typed-input example for a Job and its versioned SDC StructureMap binding. Its wording is not mandatory practice content. |
This release contains no Questionnaire instance classified as a maintained definition. The two anchor identities use definition publication metadata so their canonicals remain available; that metadata does not make them complete forms.
The canonical namespace states who owns the content:
| Namespace | Questionnaire content |
|---|---|
https://fhir.cognovis.de/praxis/ |
This IG's structural contracts, statutory anchor identities, and clearly labelled examples. It does not publish practice wording or KBV form wording. |
https://fhir.cognovis.de/<content>/ |
Maintained questionnaire definitions and guideline-derived content from a separate content package that depends on this IG. |
https://<installation-domain>/fhir/ |
Installation-owned questionnaires, local wording, forks, maps, selected defaults, and responses. |
An installation customizes a published questionnaire by creating a new
canonical and version in its own namespace and naming the source with
Questionnaire.derivedFrom. It does not edit a published questionnaire in
place.
The authoritative publication-rights classification is maintained in fhir-management: Private-content classification.
