Questionnaire Content Boundary

This guide separates the reusable questionnaire contract from concrete questions and recorded answers.

Three layers

Layer Published contract
Profile Defines reusable structure, invariants, and mapping conventions for a class of questionnaires or responses.
Questionnaire instance Gives one concrete set of questions a canonical URL and version. Each published instance is classified below as an anchor identity, example, or maintained definition.
QuestionnaireResponse Records answers and identifies the exact source questionnaire as url\|version. It does not redefine the questionnaire.

HL7 SDC supplies the targetStructureMap mechanism used by the typed Job profile. The mapping contract remains separate from questionnaire wording.

This release contains no Questionnaire instance classified as a maintained definition. The two anchor identities use definition publication metadata so their canonicals remain available; that metadata does not make them complete forms.

Content and namespace rule

The canonical namespace states who owns the content:

An installation customizes a published questionnaire by creating a new canonical and version in its own namespace and naming the source with Questionnaire.derivedFrom. It does not edit a published questionnaire in place.
